The Patient Safety Attendant (PSA) participates in high-quality, patient-centered care by providing continuous observation and monitoring for high-risk patients. In collaboration with the patient care team, the PSA ensures the patient environment is free of obstacles and minimizes the risk of falls and accidental or intentional self-harm. The PSA may also assist with basic activities of daily living. PRN Day and Night Shifts available! Essential duties include maintaining constant attendance by remaining within the continuous line of sight of the patient at all times including when the patient is using the bathroom facilities; observing as the patient eats, drinks, and assisting as needed; assuring the cleanliness and safety of the patient’s room; accompanying the patient off the unit to tests or procedures and remaining with the patient unless instructed otherwise; identifying and promptly reporting any changes or concerns in the patient's status to the assigned nurse; listening attentively if the patient wishes to converse and encouraging the patient to share concerns with the physician and/or assigned nurse; reporting patient information and activities to the next assigned PSA during hand-off with nursing present; communicating with other team members to ensure the patient is not left alone; reporting to the assigned nurse any expression of ideas or intention to hurt self/others or intention or desire to leave the facility Against Medical Advice (AMA); avoiding giving any advice; following all Infection Control policies; completing the Environmental Safety Checklist each shift and during hand-off and having it validated by nursing, including ensuring that ligature risks are mitigated in coordination with the primary nurse; consulting with the nurse partner if the patient needs to be mobilized. Additional responsibilities for patients with suicidal ideation include remaining with the patient even if family or visitors are present; ensuring continuous line of sight and constant one-on-one visual observation; ensuring environment has limited risk factors such as accounting for all linen and removing it immediately after use; ensuring patient is in a gown with no strings; removing all patient clothing, shoes, jewelry, and belongings from the room; securing valuables; restricting visitors from giving items or bringing large bags; allowing only electric razors; removing unnecessary electrical cords; using only plastic utensils for meals and accounting for them; removing all plastic trash and linen bags; ensuring windows are not opened; removing all sharps containers, gloves, and glove boxes; and ensuring no supplies, equipment, or devices that could be used as ligatures are present. Minimum requirements include a High School Diploma, GED, or suitable equivalent and current American Heart Association Basic Life Support for Healthcare Providers (BLS-HCP) certification upon hire or obtained within 60 days of hire with prior approval. Preferred qualifications include 1+ years in a hospital setting and Workplace Violence Prevention Training.
